The Nurse Case Manager shall provide holistic patient-centered, individual and family level case management and discharge/disposition planning for beneficiaries in support of peacetime and military readiness and contingency requirements.

We also provide an excellent work schedule with hours running Monday through Friday 7:00AM to 4:00 PM.
* Some overtime may be required but not to exceed 243 hours per year
Requirements Include:
* Minimum of 2 years experience in a clinical area and /or Case Management.
* Bachelor's Degree
Nurse Case Manager Certification preferred.
If not certified a 15 month time frame is granted to obtain Nurse Case Manager Certification which will be reimbursed.
Salary is negotiable.
