Job Description:

Under the supervision of the Program Manager, the Case Manager is responsible:

• Provide crisis intervention, case management and follow-up services to homeless population
• Provide information and referral services, and social service advocacy
• Develop effective, trusting relationships with clients with a focus on facilitating their move towards
independence; share resources and information
• Perform a comprehensive needs assessment (screening and intake) for all case managed clients, and in
conjunction with the client, develop a plan that is based on client’s needs, issues and goals
• Maintain accurate documentation of client information (intake, referrals, progress notes and service delivery)
• Maintain accurate client records and statistical data (manually and via computerized database system)
• Educate community members (and potential clients) about Access Center services and the issues faced
by the population served
• Participate in on-going self evaluation, program evaluation and development/modification
• Participate in all case file, staff meetings, trainings and retreats
• Communicate effectively with supervision and in a timely manner as pertains to programmatic issues
• Report to work as scheduled
• Other duties as assigned.

Qualifications/Requirements:

• B.A. or equivalent experience
• Minimum of one (1) year experience in case management
• Demonstrated knowledge of issued faced by population served
• Knowledge of social service agencies, especially on the Westside, including access to government benefits/
Welfare system
• Experience working in a community-based setting
• Commitment to improving the community through empowerment and dignity
• Skilled in non-violent crisis intervention
• Ability to self-motivate
• Computer literate; able to use computerized database system for client file management
• Ability to work as a team member
