Homeless Services Case Manager - Santa Monica, CA

Job Description:

Responsibilities:
Provide crisis intervention, emergency and case management services to homeless individuals. Develop effective, trusting relationships with clients with a focus on facilitating their move towards independence; share resources and information. Perform a comprehensive needs assessment (screening and intake) for all case managed clients, and in conjunction with the client, develop a plan that is based on client’s needs, issues and goals. Provide information and referrals services, and social services advocacy. Accurate documentation of client information, service delivery, goal plan, progress notes, etc. in computerized. Database and via manual records. Network with agency and outside service providers, ensuring client access to appropriate resources. Education community members, including potential clients, about Access Center services and service hours, as well as issues faced by population served. Participate in on-going self evaluation, program evaluation and development/modification. Attend all staff meetings, trainings and retreats as scheduled. Other duties as assigned.

Qualifications/Requirements:
B.A. or equivalent experience. Minimum on one (1) year in case management. Demonstrated knowledge of issued faced by population served. Knowledge of social service agencies, especially on the Westside, including access to government benefits/Social Security System. Experience working in a community-based setting. Commitment to improving the community through empowerment and dignity.
Skilled in non-violent crisis intervention. Ability to self-motivate. Computer literate; able to use computerized database system for client file management. Ability to work as a team member. Reports to the Program Manager
