Job Description:
Responsibility:
Provide case management and emergency services to clients that have co-occurring or multiple diagnosis of mental illness and substance addiction.
Refer target clientele to appropriate services (e.g. Edelman Westside Mental Health Center, Venice Family Clinic, Clare, UCLA, etc.), per guidelines of the Community Bridges Integrated Services for Homeless people.
Develop effective, trusting relationships with clients that facilitate provision of case management services.
Co-facilitate engagement/retention and/or 12-step groups with Dual Diagnosis team member.
Provide case management and aftercare/follow-up services to clients placed in permanent housing through the Community Bridges Team.
Provide information, referral and social service advocacy as needed.
Maintain accurate records and statistical data (manually and via Client Track system), as well as monthly data client tracking, Quality of Life Survey and other data as required.
Provide case management training and on-going support to case managers.
Monitor client files/records (hard and computer copies).
Collaborate with Community Bridges Program team members and outside agencies as necessary.
Participate in all case file, staff meetings, trainings, and retreats.
Participate in on-going self evaluation, program evaluation, development and implementation.
Work in collaboration with Community Bridges Outreach personnel in identifying and engaging clients into Community Bridges Program. Other Duties as assigned.
Qualifications/Requirements:
BA or equivalent work experience.
Demonstrated knowledge of and experience in dealing with issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence).
Experienced in group facilitation.
Knowledge of social service agencies, especially on the Westside, including government benefits/Welfare system.
Skilled in non-violent crisis intervention.
Computer literate – able to use Client Track system for client file management.
Ability to self-motivate and work as part of a team.
Bi-lingual English-Spanish preferred.
Great organizational skills a must.
